The Health and Wellbeing Practitioners (HWP), work within your local community to help individuals suffering with long-term physical and mental health conditions.
We support, encourage, and advise people to find ways to make small sustainable lifestyle changes, which can enable and effect the way they are able to better self-manage their conditions.
Implementation of lifestyle have been shown to lead to a better quality of life with reduced pain, raised confidence and less stress and low mood.
HWPs will support with chronic pain, depression and anxiety, diabetes, weight loss, and any other long-term condition which are impacting an individual's ability to properly carry out their normal daily routines.
This is a non-medical service, and our HWPs will provide tailored one to one support for any individual that approaches the service for help. This support can be face to face, or over the phone. HWPs will listen to what your needs are, your concerns, and how you feel you are able to sustain any change.
They will approach your support with a holistic point of view, as it has been recognised that in most cases, there are multiple issues restricting a person's ability to self-manage long term-conditions.
You can ask your GP or any health professional to refer you to the service, or you can enquire at the Hub by approaching a member of staff, and ask to speak to a HWP.
