'People Keeping Well' is sometimes known as Social Prescribing. It's all about connecting to a range of local services to improve general health and wellbeing. Locally, this is being delivered through our 'People Keeping Well' network of community partners who work with a wide range of people living or working in our community. Each partnership is different, depending on local needs, but might include other voluntary groups, libraries, local forums, GPs, tenants and residents' associations, faith groups, food banks, local Councillors, and neighbourhood Police Officers. Woodhouse and District Community Forum are the lead organisation for this project in the South East of Sheffield.
Woodhouse and District Community Forum has the experience of delivering a programme of health and wellbeing activities for over 15 years.
We are the lead partner for the People Keeping Well initiative, which, when using support and prevention activities, allows people to live well in their own homes.
Our focus is on all areas of health and wellbeing, such as: Emotional, Physical and Mental.
We have an excellent reputation for delivery, achievements, and results, including Community Dementia Information Service, Mental Health support, Volunteering, Health and Wellbeing Practitioner Service, Healthy Activities, Woodhouse Community Library, and Community Garden Project (a list of all our activities and contact details can be found in our Health & Wellbeing Activity List).

Our Health and Wellbeing Practitioner service, offers one to one confidential support for those who want to either change, or lead a healthier lifestyle through stopping smoking, alcohol reduction, weight management etc. We also have Chronic Pain specialists to help manage your conditions.
